Hospitalization Insurance

If you are taking 6 or more credits (3 or more credits in summer) and are a degree-seeking student, you are required to have hospitalization insurance. The University-sponsored hospitalization insurance will automatically be charged to your student account unless you provide evidence of a current hospitalization policy (insurance company name and policy number). If you do not provide this information, you will be charged for insurance. You may receive a full refund by returning the "Student Hospitalization Insurance" form (available at http://policy.umn.edu/groups/ppd/documents/form/otr290.pdf)
to one of the student service centers or by accessing your student record through the internet by the end of the first week of classes. This information is subject to periodic audits. Providing inaccurate or false information may result in unexpected charges. It is also a violation of the Student Conduct Code.

Note: You do not need to provide hospitalization insurance coverage information if you are enrolled in fewer than 6 credits (fewer than 3 credits in the summer) or are not enrolled in a degree program.

Boynton Health Service administers the student insurance plan at the University of Minnesota Rochester. This plan is known as the University-sponsored Student Health Benefit Plan (SHBP). The plan offers the same complete benefit design for eligible students, and their dependents, as provided to students on the Twin Cities and Duluth campuses. Eligible students will be automatically enrolled in the SHBP and billed $907/semester.  Students in Academic Health Center programs, including nursing, will be billed $1,164/semester. Eligible students who do not wish to purchase the SHBP plan must provide proof of other hospitalization insurance at the time of registration (insurance company name and policy number) or update insurance information online by the end of the first week of classes at http://www.bhs.umn.edu/insurance/rochester.

Immunization Clearance

The University requires all students born after December 31, 1956, who take more than one University class, to provide documentation of two immunizations for measles, mumps, and rubella; and a tetanus-diphtheria immunization within the last ten years. Currently, the tetanus-diphtheria immunization is being waived due to a shortage of the vaccine. This documentation must be entered on a Student Immunization Record and must include the month and year each immunization was given. Forms are available at UMR Student Services. Students who graduated from a Minnesota high school in 1997 or later will be exempt from this policy.

Check with your medical or high school immunization records. All students are legally required to provide information on immunizations or exemptions; students who fail to provide the required information will have a hold placed on their records and will not be allowed to enroll for classes. (The law does allow for an exception based on age, or documented medical or conscientious exemptions.) If you need any of the required immunizations, call your family doctor for an appointment.
